A teacher-coach who is hired and school is awarded funding to start an approved before school or after-school activity must maintain an "on-site" CHAMPS Program Binder. The CHAMPS Program Binder must be located in the space or location of the activity. The Binder must also be readily available for Office of School Wellness Program staff and principal (rating officer) to review its content.


The CHAMPS Program Binder is comprised of the following program staff documents, parent and student forms, and planning and preparation resources:


Program Staff Requirements:

  • Resume
  • Cover Letter
  • Completed OP175
  • Copy of the AED/CPR certification
  • Copy of the Concussion in Sports certification
  • Copy of NYC DOE teaching license or other proof of teacher certification
  • Copies of per session timesheets and timecards
