The Office of School Wellness Programs will assign a Physical Education teacher who is trained to conduct the assessment. On the day of the assessment, please make sure the documents listed below are available for review:

  • Parental consent
  • Present Levels of Performance and Individual Needs page of the student’s IEP
  • Any relevant medical documentation

For the assessment, the student should wear sneakers and comfortable clothing. A member of the school staff must be present during the assessment. A location for the Test of Gross Motor Development 3 (TGMD-3) should be designated. The gymnasium or other safe, private space with 60 feet of floor space is needed for the running component. A hallway may be used for the running component if the school's designated PE space is not available.


The school staff must upload the assessment results and determination in SESIS in a timely manner. The IEP committee, in consultation with the certified PE teacher or the assigned APE teacher, will determine goals for the student and enter them in SESIS. The certified PE teacher assigned to conduct the assessment will be available to support the decision on the need for APE, and answer questions about appropriate goals for the student.
