Students with short term medical conditions that do not have a 504 Plan are not assessed for Adapted Physical Education. Students with short term medical conditions should provide an individual certificate of medical limitations from a health care provider (physician, nurse practitioner or physician's assistant). The limitations form indicates the areas of the PE curriculum that the student is able to participate and any temporary modifications they may need. Schools may review and modify the NYSED Sample Medical Certificate of Limitations (opens in new window) to fit their program.
