IMPORTANT MESSAGE: Before starting the application, the principal must CONFIRM the nominated teacher-coach has a valid AED/CPR and Concussion in Sports (CIS) certificate already uploaded to the WellnessHub. Next, the principal will apply and request CHAMPS funding using the CHAMPS Manager App icon using their NYCPS login username and password. Note, that principals WILL NOT be able to enter teacher-coach certificate information while using the CHAMPS Manager App. 

The principal will be able to submit a completed Spring application for the Office of School Wellness Programs to screen and score when they:

  • Nominate a NYCPS teacher with valid AED/CPR and Concussion in Sports (CIS) certificates.
  • Confirm teacher-coach certificates are valid and uploaded to the Wellness Hub (opens in a new browser window)
  • Apply and request CHAMPS funding using the instructions listed below.


Steps for the Principal to Apply and Request CHAMPS Funding:

  1. Login to the Wellness Hub (opens in a new browser window)
  2. See Wellness Hub homepage, under Internal Apps, and click on the CHAMPS Manager app icon.
     
  3. See the CHAMPS Manager app sidebar, and click on "Apply for Program." Apply for Program circled on the CHAMPS Manager homepage. 
  4. See School Information Section: Confirm all information is correct and update the payroll secretary's contact information by typing their NYCPS email address and phone number. Next, click on the "Validate Payroll Secretary" button.  
  5. See Season Information Section: Select the season you wish to request funding from the drop down box. The number of programs you can apply for is defaulted to 1. 
  6. See Program Information Section: Choose a grade-specific or developmentally appropriate activity for your school community to attend before or after-school. See Activities List (opens in new window).
    1. Activity: Choose the activity from from the drop-down menu.
    2. Session: The "Session" is when the planned activity will take place. The session defaults to "Both" which will allow the teacher-coach to conduct CHAMPS activity in the morning (AM) and afternoon (PM). However, if you will exclusively run your activity in the morning (AM) or afternoon (PM) only, then choose AM or PM.
    3. Coach: Select the nominated "Coach" (full name) from the dropdown menu and add "Coach Cellphone" to complete the program information section. 
      • Coach with valid certificates: When you select your DOE licensed-nominated coach, their information saved to the WellnessHub will populate the "Coach File Number, Coach AED/CPR Expiration Date, and Coach CIS Expiration Date" sections. 
      • New coach with certificates: If a teacher-coach you nominated is not listed, click the "Add New Coach" check box and type in the DOE email which must include an "@schools.nyc.gov address." Next, click add.
      • Coach missing certificate(s): If a teacher-coach selected from the drop down menu DOES NOT populate the "Coach File Number, Coach AED/CPR Expiration Date or Coach CIS Expiration Date" fields - it means they did not upload the required certificates to their Wellness Hub profile (opens in a new browser window). Logout from the app and share instructions for uploading certifications (opens in a new browser window) with the teacher-coach. Without the certificates uploaded to the WellnessHub, the principal cannot complete the application. Login at a later time, but before the application deadline after valid certificates are uploaded to the WellnessHub.
    4. Submit Button: After all fields are filled, click the "Submit Application" button. The application will now be saved in the "Submitted Program Applications" at the bottom of the page for a Spring season. 
    5. Changes to Application: If you wish to make any changes to your application, you must make them before the application period ends. After the application period ends at 5:00 p.m. on March 8, 2024, there will not be an opportunity to apply and request funding or participate in CHAMPS for the season.
