1. Log into the Wellness Hub (opens in a new browser window)using your DOE email address ending in '@schools.nyc.gov' and the corresponding password.
    1. Read the What should I do if I cannot sign in to Wellness Hub Support Article (opens in a new browser window) if you are having issues signing into the Wellness Hub: 
  2. Click the profile icon in the top right-hand corner and select "My Profile" to open your Staff Profile.
  3. Open the "Go to" menu in the top right corner (under the profile icon), and open up the "Concussion and AED/CPR Certificates" page. 
  4. The "Concussion and AED/CPR Certificates" page has two sections. 1) a section for you to upload a new Concussion or AED/CPR Training Certificate and 2) a section with a table listing all previously submitted Concussion and AED/CPR Training Certificates (so you can review your certificates that have been successfully uploaded into Wellness Hub).
    1. How to navigate the section where you can upload a new Concussion or AED/CPR Training Certificate:
      1. To upload a new Concussion or AED/CPR Training Certificates, you must provide the following 3 required pieces of information then press the “Submit Certificate” button.
        1. Indicate if the Certificate is a Concussion or AED/CPR Training Certificate by selecting the radio box next to the correct answer (note, Concussion Training Certificate is selected by default, so make sure to change it to AED/CPR Training Certificate if necessary).
        2. Provide the training date that is listed on your Concussion or AED/CPR Training Certificate, by clicking the calendar icon to reveal a date picker, and then selecting the date of your training.
        3. Upload your certificate by clicking the "Browse" button
        4. Press the "Submit Certificate" Button. 
          1. You are required to enter all 3 pieces of information in order to submit a certificate. If you do not provide all 3 pieces of information, you will not be able to submit a certificate.
          2. By pressing submit, you are affirming that all the information you provided above aligns with the information on your certificate.
    2. How to navigate the section with a table listing all previously submitted Concussion and AED/CPR Training Certificates (so you can review your certificates that have been successfully uploaded into Wellness Hub). 
      • Within this table, you will find the following information for all Concussion and AED/CPR certificates you have uploaded into Wellness Hub:
        • Type of Certificate: this will indicate if you labeled the certificate as a AED/CPR or Concussion certificate when you uploaded it. 
        • Training Date on Certificate: this will indicate the training date you manually entered into the above form when you submitted the certificate (the system does not capture this by scanning the date from the document you uploaded) 
        • Date Certificate will Expire: this will indicate the date that is 2 years from the training date you manually entered into the above form when you submitted the certificate.
        • No of Days until the Certificate Expires: this will indicate the number of days from todays date until the Expiration date (as determined by 2 years from the training date you manually entered into the above form when you submitted the certificate).
        • Link to Certificate: Click the "Download" link to open the certificate you uploaded that aligns to the information in the row of the table.
        • Date Certificate was Uploaded: this will indicate the date that the certificate was uploaded into the Wellness Hub. 
        • Click to delete the Record: A "delete" button is provided in case you want to delete a document that uploaded in error (for example, if you accidentally uploaded a CTLE Certificate as a AED/CPR certificate).
          • Once you delete a certificate, that certificate will be removed from the system, so be sure you want to delete a certificate before you do so.
