The Office of School Wellness Programs will screen, score, and approve CHAMPS applications. Funded schools will receive a "School Award Letter." Once the school verifies their school is an approved funded program or "Recipient" the principal can confirm the Teacher-Coach. 


Steps to Confirm Teacher-Coach:

  1. Login to the Wellness Hub (opens in a new browser window) 
  2. On the Wellness Hub homepage, under Internal Apps, click on the "CHAMPS Manager app icon."
    The CHAMPS Manager icon is circled on the Wellness Hub.
  3. Coach Column: Find the teacher-coach to confirm under the "Coach Column" and scroll to the right to the last two rows on the table.
     
  4. Confirm Coach: Once you have reviewed and confirmed your teacher-coach certificate dates indicated are valid through the end of the season scroll back to the left, and check the box next to the name of the teacher-coach you wish to confirm. Confirm Coach checkboxes are circled.
  5. Submit Button: Scroll to the bottom of the page and check the verification checkbox that reads, “The school’s principal has verified that the teacher-coaches’ certification dates are accurate,” and click submit.Checkbox and Submit button are circled.
