1. The principal nominates a NYCPS licensed teacher-coach with valid certificates uploaded to the WellnessHub PRIOR to the principal applying online. 

• AED/CPR Certificate must be from Emergency Skills Inc. (ESI), which is authorized by the American Heart Association. ESI is the only DOE-contracted vendor for AED/CPR training. The certificate must remain active and cannot expire before the end of the season coached. 
  
  • Winter Season: Monday, November 6, 2023, through Friday, February 16, 2024
  • Spring Season: Monday, April 8, 2024, through Friday, June 14, 2024

• Concussion in Sports (CIS) Certificate is from the National Federation of State High School (NFHS) Association. The certificate must remain active and cannot expire before the end of the season coached. 
  • Winter Season: Monday, November 6, 2023, through Friday, February 16, 2024
  • Spring Season: Monday, April 8, 2024, through Friday, June 14, 2024

  
  

2. The principal plans to complete the “one-time” CHAMPS application for season(s) described in the application instructions, when launched by Office of School Wellness Programs. 

• Eligible school principals will receive an email from Office of School Wellness Programs borough support team with detailed instructions on how to apply and request program funding using the CHAMPS Manager App.

• Completed CHAMPS application by the set deadline; including valid certificates are screened by OSWP staff as part of the comprehensive school selection criteria. 

3. The principal understands that completing an application does not guarantee a school will receive CHAMPS funding. CHAMPS application, including valid certificates, are screened by members of the Office of School Wellness Programs staff as part of the comprehensive school selection criteria. Schools whose applications are approved will be notified and receive CHAMPS funding via “School Award Letter” with detailed instructions.

4. The CHAMPS funding is subject to budget availability for the 2023-2024 SY.
